*Disclaimer: The Chiropractic Adjustment Wellness Membership Program is offered to self-paid relatively healthy people with minor musculoskeletal aches and stiffness (at treating physician’s discretion), who want to further improve and maintain their optimal health.
Services Guidelines and Refund Policy
- The initial payment for a minimum of two months of selected membership program is required to become a member.
- After the first two months of starting the Chiropractic Adjustment Membership Program, the monthly payments are required in the beginning of each month to maintain the active membership status.
- The unused visits do not roll over to the next month and cannot be used for the following months.
- The membership can be canceled within the first week and after two months of starting the membership program, and a 100% refund will be issued for unused visits.
- The membership can be canceled by coming to the office in person or by sending a signed cancellation form via e-mail.
- The membership can be restarted at any time after the cancellation of the initial membership by making a payment for a minimum of two months of selected membership program.
